    REPORTS TO: Executive Director

    Program Planning & Delivery

    • Plan, schedule, and run all of Hospice Georgina's programs (e.g., Ageless Grace, Art Heals, Caregiver Support Group, Cancer Support Group, Chair Yoga, Coffee & Chats, Creative Connections, Creative Paint Class, Day Hospice, Georgina Parkinson's Support Group, Line Dancing, Movement & Meditation, Silver Wings Virtual Reality, Women's Healing Circle, and others as offered).
    • Develop and implement program plans and calendars to meet organizational goals and community needs, from initial concept through execution and evaluation.
    • Promote programs to clients and the community, and actively reach out to enroll and re-engage clients so program spaces are filled and attendance is maintained.
    • Prepare, set up, and facilitate or oversee facilitation of program sessions, coordinating guest speakers, facilitators, and any materials or equipment needed.
    • Recruit, place, and coordinate volunteers to support and help run programs, ensuring each program has the volunteer coverage it needs.
    • Track program attendance, participation, and outcomes, and use this information to evaluate and improve programming.
    • Ensure the provision of cost-effective client services and programs.

    Client Referrals, Intake & Case Support

    • Determine the appropriateness of referrals for in-home visiting and program services, and process client intake from first contact through assessment.
    • Assess clients for service needs and monitor service delivery.
    • Conduct regular client engagement and follow-ups to ensure programs and services meet expectations.
    • Ensure all client forms, including Service Agreements and consents, are completed, signed, and maintained.
    • Redirect referrals that fall outside Hospice Georgina's scope to appropriate community resources, in a professional manner.
    • Support and advocate on behalf of clients as requested.

    Volunteer Management & Matching

    • Recruit, screen, and hire volunteers for in-home visiting and program roles.
    • Match trained volunteers to clients based on needs, interests, and compatibility.
    • Lead volunteer orientation and training, including in-services within the framework of Hospice Georgina standards.
    • Provide ongoing support, coaching, and resources to volunteers.
    • Maintain clear communication with volunteers, addressing concerns or program-related issues promptly.
    • Discuss any volunteer-related concerns or complaints with the Executive Director as needed.

    Community Awareness

    • Contribute to community awareness of Hospice Georgina's services as part of the broader team effort (this role is not primarily responsible for outreach, but supports it where relevant — e.g., representing Hospice Georgina at occasional community events or meetings).

    Administrative & Reporting

    • Maintain accurate records for clients, volunteers, and program activities in compliance with organizational policies.
    • Support financial accountability by maintaining program budgets.
    • Prepare reports and data as required by the Executive Director, funders, and the Ministry.
    • Submit a summary of coordination activities for monthly Board meetings and prepare an annual report for the AGM.

    Compliance & Confidentiality

    • Ensure client and volunteer files are securely stored and locked at the end of each day, per Hospice Georgina policy.
    • Maintain strict confidentiality and adhere to Hospice Georgina's standards at all times.
    • Provide a clean criminal reference check.

    SKILLS & QUALIFICATIONS

    • Bachelor's degree in Social Work, Health Sciences, Community Development, or related field (or equivalent experience).
    • Experience in program coordination, client intake/referral processes, and/or volunteer management.
    • Experience in the non-profit sector.
    • Strong organizational and time-management skills, with the ability to manage multiple priorities and run programs independently.
    • Excellent interpersonal and communication skills, both verbal and written.
    • Proficiency in Microsoft Office Suite and Alayacare or equivalent client/volunteer management software.
    • Experience with budget management and report writing is an asset.
    • Knowledge of community resources and networks within the Georgina area is preferred.
    • Valid Driver's License: A valid driver's license and access to a reliable vehicle are required for travel within the community.

    ONBOARDING AND PROFESSIONAL DEVELOPMENT

    New hires will participate in an onboarding program that includes:

    • Orientation to Hospice Georgina's mission, values, and policies.
    • Mandatory Hospice Palliative Care of Ontario training.
    • Training on client and volunteer management systems.
    • Introduction to key community partners and resources.
    • Ongoing professional development opportunities, including workshops, webinars, and access to relevant certifications.

    HOURS OF WORK

    Monday to Thursday: 8:30 AM – 4:30 PM. Occasional evening and weekend availability required for community events or program needs.
