    Grief & Bereavement Coordinator Job Description

    Location: Sutton West, Ontario
    Reports to: Executive Director
    Employment Type: 32 hours/weekly Monday – Thursday 8:30 am -4:30 pm

    DUTIES AND RESPONSIBILITIES:

    As a Grief & Bereavement Coordinator, you’ll support individuals and families facing grief and loss by providing emotional support, connecting them to resources, and coordinating volunteer-led services. You will ensure that our services are welcoming, client-centered, and responsive to the unique needs of every individual while building strong partnerships within the community.

    Client Support & Case Coordination

    • Provide grief and bereavement support to individuals and families through compassionate conversations, resource referrals, and follow-ups.

    • Conduct intake and needs assessments to understand each client’s unique circumstances.

    • Match clients with trained volunteers and maintain ongoing contact to ensure high-quality support.

    • Liaise with family members, health care providers, volunteers, and community partners to ensure seamless care.

    • Ensure accurate and confidential documentation of all client interactions.

    • Provide case management of client needs through intake, needs assessment, feedback, and ongoing supportive contact.

    • Ensure that clients understand the role and limitations of Hospice Georgina services.

    • Provide one-to-one bereavement support to clients as needed.

    • Prepare and ensure accurate client data.

    • Process the necessary paperwork in the management of each case.

    • Provide ongoing monitoring of clients and conduct client follow-up as needed.

    Quality Assurance

    To meet quality standards for Grief and Bereavement services including reports and data as requested; maintain ongoing case management and volunteer documentation, including:

    • Prepare and ensure accurate client data is updated electronically.

    • Provide reports and data as required and requested by the Ministry, funders, and the Executive Director including a summary report to the Board for review at monthly Board meetings and an annual report for the Annual General Meeting.

    • Ensure that print and website materials are accurate and current.

    • Work with the Executive Director to plan, develop, implement, evaluate, and enhance services for clients.

    • Provide general support for strategic planning, accreditation, quality assurance, fund-raising, and other activities of Hospice Georgina

    • Other duties as assigned.

    • Provide a clean criminal reference check.

    KNOWLEDGE, EXPERIENCE, SKILLS AND PERSONAL ATTRIBUTES:

    Education & Experience

    • Psychotherapy

    • Social Service Worker Diploma

    • Bachelor or Master of Social Work

    • Direct experience providing grief and bereavement or psychosocial support.

    • Knowledge of community resources and services in the health and human services sector.

    • Experience working in culturally diverse communities is an asset.

    • ·Previous experience in volunteer coordination is considered an advantage.

    • Demonstrates strict confidentiality in the performance of duties.

    • Experience working with youth, seniors, and aboriginal communities is an asset.

    Job Type: Full-time

    Pay: $30.00 per hour

    Expected hours: No more than 32 per week
