• Rapid Screening Kits Order Request Form - Version 3

    Please fill out the form below if you would like to order Rapid Screening Kits for your business. In order to receive them you must attend a a virtual information session.

    Attending one of these sessions (virtually) is mandatory in order to pick-up rapid tests for your business. During this session, we will provide you with information about the program, show you a video about the tests, explain the agreement that you will need to sign and answer any questions you may have. You will receive an e-mail from the Georgina Chamber of Commerce with a Zoom link for the information session.

    Following the mandatory session, an agreement will be emailed to you. You will be required to submit a signed copy of the agreement at pick-up, before receiving your tests.

    Please DO NOT visit the pick-up locations until you have heard from us.
